Once you submit your form, here is how the process works:
Watch for an email from our team. We will reach out to find an appointment time that works for your family and gather some initial information. If you don't see our email in your inbox, please check your promotions or junk folder.
Complete your patient portal registration. After your intake is scheduled, you will receive a patient portal invitation. In the portal, you will complete required registration and intake forms. If you plan to use insurance, you will also upload the front and back of your insurance card.
Submit your physician referral (if applicable). Upload it to the portal or have your provider fax it to 1-866-342-9573. A referral is required for speech, OT, nutrition, and ABA services if you are using insurance.
Add a payment method. Put a payment method on file in the portal and complete the required financial agreement forms.
