Once we receive your request someone will reach out to you to get more information and discuss benefits. If using insurance we will run a benefits check prior to your first appointment. A referral/order is required for insurance-based speech, OT, and PT and can be faxed to us at 1-866-342-9573.
Our confidential patient portal can be used to communicate with your child’s provider; make, change, or cancel appointments; and pay your bill. We require a credit card on file at the time of booking.
Once scheduled you will receive an email invitation to our portal to complete paperwork. Please complate in a timely fashion for our therapists to review.
For insurance purposes, you can add your referral from your physician one of three ways: add it under documents in our portal, bring it with you to your evaluation, or have your referring physician fax it to us.
